What is refractive lens exchange?
Also called lens replacement surgery or clear lens exchange, RLE uses the same lens-replacement approach as cataract surgery.
The difference is the reason for treatment: RLE primarily aims to correct your prescription and reduce glasses use, rather than remove a cataract affecting your vision.
It permanently removes your natural lens and any remaining ability it has to change focus. Artificial lenses work differently, so understanding the trade-offs matters.
Who might consider it?
RLE may be considered when you want to explore less reliance on glasses, particularly as your natural reading focus declines.
Your consultant considers your prescription, cornea, retina and overall eye health, alongside activities such as reading, screen use and night driving.
Age alone does not determine suitability. For younger people or those with high short-sightedness, the risks and alternatives need particularly careful discussion.

Choose a lens with realistic expectations
No implant is best for everyone. Your consultant explains which designs may suit your eyes and the tasks you want to do with less reliance on glasses.
Monofocal
Provides one main focus distance, chosen as part of your treatment plan.
Glasses are usually needed for other distances. Your consultant can explain whether a different focus target for each eye is appropriate.
Toric correction
Designed to reduce suitable regular corneal astigmatism.
Toric correction can be incorporated into different lens designs. It does not, on its own, provide a full range of focus or guarantee freedom from glasses.
Extended depth of focus
EDOF lenses aim to extend the range of useful focus, often supporting distance and intermediate tasks such as screen use.
Reading glasses may still be needed, especially for small print or close work. Optical side effects vary with the lens design.
Multifocal or trifocal
Designed to provide useful vision at several distances and reduce glasses use.
Trade-offs can include halos, glare or reduced contrast, particularly in dim light. These lenses are not suitable for every eye.
Understanding your alternatives
You do not have to choose surgery
Continuing with glasses or contact lenses is an alternative. Depending on your eyes, laser vision correction or a phakic lens implant that leaves your natural lens in place may also be worth discussing.
Your consultant explains the relevant options and can advise if another approach is more suitable. RLE is a permanent decision; take time to consider it.
Already have a cataract?
If a cloudy natural lens is affecting your vision, cataract surgery may be the more relevant treatment pathway.
The operation is similar, but the clinical reason for surgery and your treatment plan are different.

Recovery, limitations and risks
RLE is surgery inside the eye. Its potential benefits should be weighed against the risks, especially when your natural lens is still clear.
What to expect afterwards
Vision is often blurred at first and may settle over several weeks. With some implants, adjusting to the visual effects may take longer.
Use your prescribed drops and follow your surgeon’s advice about eye protection, exercise and returning to work.
You may still need glasses, and further treatment may sometimes be required. The implant does not prevent other age-related eye conditions.
Driving: resume only when your vision meets legal requirements and you have followed your surgeon’s advice.
Possible complications
- Infection, inflammation, bleeding or raised eye pressure.
- Swelling at the back of the eye.
- Retinal tear or detachment.
- Implant position problems or a different focusing result from the one intended.
- Halos, glare, reduced contrast or difficulty adapting to the implant.
- Rarely, permanent sight loss.
Retinal risks need particular consideration in highly short-sighted eyes. Your surgeon will explain the risks relevant to your eyes.
The membrane behind your implant can become cloudy months or years later. This is posterior capsule opacification (PCO), which may be treated with YAG laser capsulotomy when appropriate.

Is RLE the same as cataract surgery?
The lens-replacement approach is similar. Cataract surgery treats a cloudy natural lens affecting vision. RLE primarily aims to reduce dependence on glasses or contact lenses when there is no significant cataract.
Will I be completely free from glasses?
No lens guarantees complete freedom from glasses. Your likely result depends on your eye health, prescription, implant and visual tasks. Glasses may still be needed for reading, night driving or fine detail.
Is there an age limit for lens exchange?
RLE is often considered after natural reading focus has declined, but age alone does not determine suitability. Removing a natural lens is permanent. Younger people and those with high short-sightedness need particularly careful discussion of alternatives and retinal risks.
How much does RLE cost?
Consultation costs £220 and is charged separately. RLE starts from £2,795 per eye, with the final price depending on the implant and your clinical needs. You receive a personalised treatment quote before proceeding.
What are the alternatives to RLE?

Is lens replacement surgery painful?
Local anaesthetic is used to numb the eye. You may notice pressure, touch or changes in light. Tell your surgical team if you feel discomfort, and discuss any anxiety before your operation.
Can both eyes be treated?
If treatment is appropriate for both eyes, your consultant will discuss the timing and sequence. Do not assume both eyes will be operated on during the same visit.
How long does recovery take?
Vision may be blurred initially and can take several weeks to settle. Adjustment to some implant designs can take longer. Follow your surgeon’s advice about eye drops, work, exercise and driving.
Can I develop cataracts after RLE?
A cataract cannot develop in the removed natural lens. However, the membrane behind the implant can become cloudy, causing posterior capsule opacification (PCO). YAG laser treatment may help when appropriate. Other eye conditions can still affect your vision.
Do I need a referral?
